Coach Requirements for Mt. Lebanon Soccer

To become a coach with Mt. Lebanon Soccer, please complete the following steps:

1. Establish an Affinity Account
This is the PA West system used to store and manage coach clearances.

2. Upload the Required Documents into Your Affinity Account:

o Child Abuse Clearance - Go to www.compass.state.pa.us/cwis/public/home

o State Police Clearance - Go to epatch.state.pa.us

o FBI Clearance or Affidavit – Go to https://www.dhs.pa.gov/KeepKidsSafe/Clearances/Pages/FBI-Fingerprinting.aspx or volunteer_affidaviteditrev.pdf (pawest-soccer.org)

o Photo of Yourself – A headshot is required to be uploaded to Affinity to complete your credentials.

o Free Introduction Coaching Course
